Case Management & Health Education
PCCCF provides case management, counseling, and health education for seniors and adults living with chronic or cognitive conditions (e.g., diabetes, heart disease, strokes, substance dependency, dementia, Alzheimer’s). Community Health Workers help clients and caregivers navigate care and improve outcomes. Services include comprehensive assessments, coordinated care to prevent premature institutionalization, medical & mental-health referrals, and domestic violence/abuse referrals.

Aging, Senior & Disabled Adult Services
We help older adults and people with disabilities live independently and stay healthy. Support includes one-stop information and assistance; help with Medicaid, Medicare, Social Security, and legal matters; applications for benefits; proactive follow-up; and transition support to prevent institutionalization.

Medical & Mental Health Services
Short-term counseling, referral coordination, and caregiver support to help clients access primary care, behavioral health, and community resources—reducing barriers and improving continuity of care.

Vocational & Employment Services
Pathways to work through job readiness, skills training (healthcare, office, manufacturing), vocational guidance, individualized reemployment plans, job search assistance & coaching, aptitude/skills testing, and supports for older workers.

Career Enhancement Programs
Placement into training and re-training programs, advanced occupational and certificate pathways, and targeted skill-building aligned with employer needs—strengthening New York’s workforce and reducing future unemployment.

Naturalization Services
PCCCF supports immigrants on the path to U.S. citizenship with Creole-language classes and guidance through the naturalization process—promoting active civic participation.
