Pest or mold issues, lead paint or pipes, no heat, broken stoves, missing smoke detectors, water leaks or poor ventilation—PCCCF helps identify hazards and connect you to the right solutions and benefits.

Available Benefits

  • Up to $10,000 for accessibility, safety improvements and pest/mold removal
  • More than $13,500 in rental arrears or future rent assistance
  • Over $4,700 for utility setup and past-due payments
  • Over $5,000 for community-based transitional housing support
Transportation assistance for medical and social care appointments

Transportation Assistance

Transportation should never be a barrier. PCCCF helps eligible members get to healthcare, housing and social-service appointments safely and on time.

Who Is Eligible for Transportation Assistance?

Medicaid members receiving Enhanced HRSN Services may qualify for transportation to:

  • Housing appointments & eviction-prevention court hearings
  • Social services & vital records offices
  • Job interviews & training programs
  • Health education & chronic disease management sessions
  • Court, parole, probation & other legal appointments
  • Food pharmacies, farmer’s markets & mobile food programs

Available Modes of Transportation

  • Taxi / Livery
  • Rideshare (Uber, Lyft, etc.)
  • Public transit assistance
Service Duration & Authorization: Available up to 6 months with possible re-authorization. Requests must be documented in the member’s Social Care Plan and assessed for cost-effectiveness. Eligibility limited to Medicaid members receiving Enhanced HRSN Services. See state Medicaid guidance for details.
Healthy food and nutrition support for medically tailored prescriptions

Medically Tailored Food Prescriptions

Personalized nutrition support to improve health outcomes:

  • Vouchers or food boxes for fresh, healthy groceries
  • Redeem at food pharmacies, pantries, grocery stores, farmers’ markets & CSAs
  • Covers 3 meals/day, 7 days/week
Note: Members receiving food prescriptions cannot also receive home-delivered meals or pantry stocking.
